Please submit the form to the MLIB Housing Department either by:
1. Hand deliver to MLIB Reception Office in McLeod Lake
2. Hand deliver to the PG Office to be forwarded to the housing office @MLIB.
3. Scan and email to housingcoordinator@bandmanagermlib.ca
4. Scan and fax it to 1- 250- 750-4420 to the attention of the Housing Department
5. Mail it to – McLeod Lake Indian Band, General Delivery, McLeod Lake B.C, V0J 2G0
